Prior to your therapy appointment please download, print off and fill in the consultation form. Please bring this with you to your first appointment.

We operate a 48 hour cancellation policy on all appointments made. We request that you give a minimum of 48 hours notice to cancel or move your appointment. Late cancellations or no shows will incur the full charge.
